HealthCore Privacy Policy

Last revised and effective as of: October 29, 2020

SCOPE OF WEBSITE PRIVACY POLICY.
Browsing our websites at https://www.healthcore.com, https://www.pediatricheartnetwork.org, and/or any sub-domains thereof (collectively, the “Website”) may not require you to register for an account or actively provide any personally identifiable information to us, but as indicated below, we and third parties may use cookies and action tags to passively collect information from visitors to our Website.

HealthCore, Inc. (“HealthCore”, “we”, “us”, or “our”) is a provider of research services that leverage HealthCore’s integrated database in designing custom data-driven research solutions to guide HealthCore’s clients in the optimal use and interpretation of data by evaluating the impact of disease, treatment and medical care on outcomes. This Website Privacy Policy describes how HealthCore handles information that we collect on the Website.

HealthCore also has an EU Privacy Policy which applies to information relating to identified or identifiable natural persons in the European Economic Area (“EEA”), the United Kingdom (“UK”) and Switzerland that HealthCore collects on the Website and through other online and offline channels in connection with your interaction with us. Individuals in the EEA, the UK and/or Switzerland should carefully read the EU Privacy Policy in conjunction with this Website Privacy Policy.

For clarification but not limitation, with respect to our U.S. operations, this Website Privacy Policy only applies to information collected through the Website and does not apply to information collected through offline and other online channels in connection with our U.S. operations. You should carefully read this Website Privacy Policy. By using the Website, you are signifying your acceptance of this Website Privacy Policy. If you do not agree to this Website Privacy Policy, you are not permitted to use the Website.

As used in this Website Privacy Policy, the terms “using” and “processing” information include using cookies on a computer, subjecting the information to statistical or other analysis and using or handling information in any way, including, but not limited to collecting, storing, evaluating, modifying, deleting, using, combining, disclosing, and transferring information within our organization or among our affiliates within the United States or internationally.

WHAT INFORMATION ABOUT ME IS COLLECTED THROUGH THE WEBSITE?
Depending on your use of Website, we may collect two types of information: personally identifiable information and non-personally identifiable information.

Personally Identifiable Information
Personally identifiable information is information that identifies you or can be used to identify or contact you. The categories of personally identifiable information that may be collected through the Website include but are not limited to your name, title and specialty, email address, telephone number, company name and industry, questions and comments and physical address.

Non-Personally Identifiable Information
Non-personally identifiable information is information, any single item of which, by itself, cannot be used to identify or contact you. The categories of non-personally identifiable information that may be collected through the Website include but are not limited to questions and comments, IP addresses, browser types, unique device identifiers, device types, what type of Internet browsing device you are using, requested URL, information such as how you arrived at our Website (including referring URL), network name, the country, state, and city where your server is located, browser language, the pages you view, the date and time of your visit, domain names, and other statistical data involving use of the Website. Certain non-personally identifiable information may be considered a part of your personally identifiable information if it were combined with other identifiers (for example, combining your zip code with your street address) in a way that enables you to be identified. However, the same pieces of information are considered non-personally identifiable information when they are taken alone or combined only with other non-personally identifiable information (for example, your viewing preferences) in a way that would not enable you to be identified.

Registering to Use the Website.
You may be required to establish an account in order to take advantage of certain features of the Website. If you wish to establish an account, you will be required to provide us with information (including personally identifiable information and non-personally identifiable information) such as name email address, telephone number, organization, affiliation, practice, title, studies of interest, state and specialty. In addition, we may obtain your personally identifiable information from you if you identify yourself to us by sending us an email with questions or comments. Also, we will have access to any personally identifiable information that you choose to share through the Website.

WHERE AND WHEN IS INFORMATION COLLECTED THROUGH THE WEBSITE, INCLUDING THROUGH THE USE OF COOKIES AND ACTION TAGS?

Information provided by you
We will collect personally identifiable and non-personally identifiable that you submit to us. As an example, we may obtain your personally identifiable and other information from you if you identify yourself to us by sending us questions or comments.

Cookies and Action Tags
We may collect information passively using “cookies” and “action tags.” By using cookies and action tags together, we are able to gain valuable information to improve the Website.

“Cookies” are small text files that can be placed on your computer or mobile device in order to identify your Web browser and the activities of your computer on the Website and other websites. Cookies can be used to personalize your experience on the Website, to assist you in using the Website, and to allow us to statistically monitor how you are using the Website to help us improve the Website. We also may use cookies to determine the popularity of certain content. In addition to cookies that we place on your computer or mobile device, cookies may also be placed on your computer or mobile device by third parties, including third parties providing analytical and other services related to the Website. In the course of providing services, such third parties could place or recognize unique cookies on your browser. You do not have to accept cookies to use the Website. Even if you initially agree to accept cookies, you can change your preferences at any time. Although most browsers are initially set to accept cookies, you may reset your browser to notify you when you receive a cookie or to reject all or some cookies. Most browsers offer instructions on how to do so in the “Help” section of the toolbar. However, if you reject cookies, certain features or resources of the Website may not work properly or at all, and you may experience some loss of convenience.

For the avoidance of doubt, this Website may use third-party service platforms. These third-party service platforms may place cookies on your computer or mobile device. If you would like to disable “third party” cookies, you may be able to turn them off by going to the third party’s website.

We may, without limitation, use the following cookies:

  • Google Analytics cookies to understand web metrics
  • Session cookies may be set by WordPress when a user logs in to certain areas of the Site. These cookies contain a user’s authentication details and the settings for the admin area interface. These session cookies expire every 15 days.
  • Pardot tracks visitor and prospect activities on certain areas of the Website by setting cookies on your browser. Cookies are set to remember preferences (like form field values) when a visitor returns to the Website. Pardot also sets a cookie for logged-in users to maintain the session and remember table filters.

Here are links to the main third-party platforms we may, without limitation, use:

“Action tags,” also known as web beacons or gif tags, are a web technology used to help track website usage information, such as how many times a specific page has been viewed. Action tags are invisible to you, and any portion of the Website or email sent on our behalf may contain action tags.

Finally, you should be aware that third parties may use their own cookies or action tags when you click on a link to their websites or services on or from the Website. This Website Privacy Policy does not govern the use of cookies or action tags or the use of your information by such third-party websites or services.

Log Files
We also collect information through our Internet log files, which record data such as user Internet Protocol (IP) addresses, browser types, domain names, and other anonymous statistical data involving the use of the Website. This information may be used to analyze trends, to administer the Website, to monitor the use of the Website, and to gather general demographic information. We may link this information to personally identifiable information for these and other purposes such as personalizing your experience on the Website and evaluating the Website in general.

Without limitation of the foregoing, an IP address is a number that automatically identifies the computer or mobile device that you are using to access the Internet. The IP address enables us to send you the site pages that you want to visit or data you want to view. The IP address may disclose the server owned by your Internet service provider. We use your IP address to help diagnose problems with our server and to support our website administration.

We may also use the information gathered through the Website (including your personally identifiable information) for statistical purposes, to perform statistical analysis of user behavior, and to evaluate and improve the Website and our business. We may link some of this information to personally identifiable information for internal purposes. Without limitation to the foregoing, we may collect non-personally identifiable information on individuals who view our site to analyze traffic patterns, site usage and behaviors associated with the Website. Personally identifiable information collected by HealthCore may be transferred from time to time to our offices or personnel, or to third parties, located throughout the world, and the Website may be viewed and hosted anywhere in the world, including countries that may not have laws of general applicability regulating the use and transfer of such information. By accessing the Website and submitting such information on it, or by otherwise submitting personally identifiable information to HealthCore, you voluntarily consent to the trans-border transfer and hosting of such information. Without limitation of the foregoing, you hereby expressly grant consent to the HealthCore to: (a) process and disclose such information in accordance with this Website Privacy Policy or our EU Privacy Policy, as applicable; (b) transfer such information throughout the world, including to the United States or other countries that do not ensure adequate protection for personally identifiable information (as determined by the European Commission); and (c) disclose such information to comply with lawful requests by public authorities, including to meet national security or law enforcement requirements. DOES THIS WEBSITE PRIVACY POLICY APPLY WHEN I LINK TO OTHER WEBSITES OR SERVICES?
Our Website may provide you with access to other websites and services. Please be aware that we are not responsible for the privacy practices of any websites or services other than the Website. IS THE INFORMATION COLLECTED THROUGH THE WEBSITE SECURE?
We want your information (including personally identifiable information) to remain secure. We strive to provide transmission of your information from your computer or mobile device to our servers through techniques that are consistent with commercially reasonable standards and to employ administrative, physical, and electronic measures designed to protect your personally identifiable information from loss, misuse, and unauthorized access, disclosure, alteration and destruction, taking into account the risks involved and the nature of the information. Notwithstanding the above, you should be aware that there is always some risk involved in transmitting information, both over the Internet and through other online and offline channels. There is also some risk that others could find a way to thwart our security systems. You are responsible for maintaining the secrecy of your unique password and account information at all times.

You should be aware that it may not be technologically possible to update, delete or change each and every record of the information you have provided to us from our system. The need to back up our systems to protect information from inadvertent loss means that a copy of your personally identifiable information may exist in a non-erasable form that will be difficult or impossible for us to locate.
